Membership dues are $25.00 a year. We welcome additional donations
at any time. All dues and donations are tax-deductible. You can apply by
downloading and mailing the form or using the online application below.
We are only able to use Paypal for payments at this time.

In order to provide both a secure environment for collecting donations and
membership dues, and the safekeeping of confidential donor personal and
financial information, it is the policy of Faith Outreach Foundation
(hereinafter referred to as "FOF") to maintain donor privacy through the
All donations or dues payments will be made by PayPal, cash, check via the United States Postal Service, or in person.
All donor information is electronically stored independently of internet access except for emails among members of the Board of Directors.
Access to donor information is limited to members of the Board of Directors.
The FOF endorses the Donor Bill of Rights created by the American Association of Fund Raising Counsel (AAFRC), Association for Healthcare Philanthropy (AHP), the Association of Fundraising Professionals (AFP) and the Council for Advancement and Support of Education (CASE).
All questions or issues pertaining to security of donor information may be directed to the FOF Board of Directors Secretary who is responsible for the maintenance of internal controls over donor security and privacy procedures.
Discontinuation of contact with FOF may be requested at any time. Such request will be immediately honored and all contact and financial information removed from FOF records. A record of all such requests will be maintained by the FOF, however, to the extent necessary for legal or liability purposes.
